Pursuant to the “KVKK-Kişisel Verilerin Korunması Kanunu” (KVKK-Personal Data Protection Law) numbered 6698 and the “Kişisel Sağlık Verilerinin İşlenmesi ve Mahremiyetinin Sağlanması Hakkında Yönetmelik” (Regulation on the Processing of Personal Health Data and Ensuring the Privacy of Personal Health Data), my personal data, sensitive personal data, health data, all kinds of personal data, including those that identify or serve to identify me, given by me verbally/written and/or electronically, HABAŞ SINAİ VE TIBBİ GAZLAR İSTİHSAL ENDÜSTRİSİ A.Ş. and its group of companies and all its subsidiaries and affiliates, to be shared with other persons and organizations where sharing is required within the scope of the relevant legislation in order to select candidates for the purpose of making an employment contract, to sign the employment contract and to provide health services in accordance with the employment contract and to plan these services; I hereby accept, declare and undertake that I consent to the processing of personal and sensitive personal data detailed below within the scope defined in the “Personal Data Protection Law” numbered 6698 and the “Regulation on the Processing of Personal Health Data and Ensuring Privacy”.

• Documents such as driver's license, src, identity card and passport containing information such as name-surname, Turkish ID number, nationality, mother's name-father's name, place of birth, date of birth, gender, tax number, SSI number, iban number, signature information, vehicle license plate, etc.
• Information such as phone number, address, e-mail address, fax number, IP address
• Personal data related to records and documents taken at the entrance to the physical space, during the stay in the physical space; camera records, fingerprint records and records taken at the security point
• Photographs and camera records (excluding records entered within the scope of Physical Space Security Information), data contained in documents that are copies of documents containing personal data
• Special quality data specified in Article 6 of the KVKK (e.g. health data including blood type, criminal record information, disability status, biometric data, etc.)
• Personal data relating to the receipt and evaluation of any request or complaint addressed to HABAŞ SINAİ VE TIBBİ GAZLAR İSTİHSAL ENDÜSTRİSİ A.Ş. and its group of companies and all affiliates
